Covered but refuse to pay...

I recently submitted a claim to standard bank credit life insurance due to an injury of my foot and I had a transmetatarsal amputation procedure done to my right foot( amputation of all toes and partial of right side of foot). I have then submitted a claim for dreaded disease/ disability. On the 12/10/2016 I contacted Craig at the insurance department to find out on progress of the claim, he then advised me that in 2008 a disability claim was paid out to me on my behalf and that the policy should have been cancelled at that time. He then spoke to his team leader and advised me that the only option was to refund my premiums. I then advised Craigh that the original policy was cancelled and in 2012 when I increased my overdraft i was issued a new policy. I then asked him how this policy was underwritten and accepted by your insurance department. He then said he will come back to me which he did so. Two days later I received an sms from the insurance department stating that the claims was in progress , then on the 24/10/2016 I contacted Timothy who advised me that the claim was rejected because it was paid in 2008. I advised him that previously I did explain to Craigh that this was a new policy and how was the underwriting done and the policy accepted which I have been paying this premium for the past four years. He then referred me to the complaints department which I did and I was given a reference number ********** 550. A week later I received a call explaining that the claim was declined again as dreaded disease is not covered under this policy and that they previously paid a claim for disability. I then again questioned this guy on how this policy was underwritten by the assessor. He then advised me that the policy was not underwritten by an assessor but was done by a claims advisor. My question is how can a reputable financial institution claims advisor tell me that this claim was previously paid from my first policy, why couldn't he pick up this information when my new policy was accepted. If this injury did not occur I would have been paying for this policy probably for the next 10 years and paid off my loan and overdraft and I would have not known that this is not a valid policy and I would have not gotten a refund from this policy. If this policy covers disability and death , why then would they want to cancel this policy and refund my premiums when I am still alive and I am having difficulty in mobilizing myself. In my opinion this claim was accepted and I honored my premiums faithfully for the past four years, therefore this claim should be honored.
