With Holding cancellation | resignation forms and memership certificate so I can change medical aids

No hospitals,Medicross, EMERGENCY ROOMS or GPs are NOT accepting this medical aid and want upfront payments. I would like to leave this medical aid but I can not reach them , they will not give me the forms to resign from the medical aid and or my membership certificate. Which I need to change to a new medical aid without having a waiting period. They are essentially blocking me from leaving a failing medical aid scheme. I also have not been able to access the member portal to verify claims have been paid, processed or rejected or download my own medical aid certificate. If anyone has left the medical aid, please can you share the forms and how you got this right? This is essentially ********* to stay and pay while we have no coverage. Rfs 73983)
