Unsuccessfully trying to terminate membership!!!

Everytime I call and ask for something to be emailed to me, I provide them with email address, even using phonetic alphabet to get the spelling right, and still have not received any email. I am trying to get a termination of membership form. Ridiculous that they don't even have a link to it on their website. I have even emailed them on *** a week ago and have still not received any response. I've called over a week ago and again yesterday. How hard is it to send one email? Don't they have the email address on file? This is absolutely ridiculous!!!
