Unpaid claim for for 4 months

Very poor service they didn't pay my path care account since last year October and now I'm getting calls from legal depart for non payment. I made a follow up with them for more than 6 times no luck. Instead getting the same lies it's with the claims department. I provided all the documents needed, no luck till today
