My name is Gary Wilder ID *** Main member Mr GA Wilder Patient Mr GA Wilder Sizwe membership ***
Dr Nay amount of R1745.05 still not paid. Practice number ***. Treatment date 30 August ICD 10 S01X59.99 Phoned Pumla at Sizwe 6 October. She asked for Alberton Hospital practice number which is *** then she gave me authorisation number NEO***A. This has still not been paid. Please let me know when it will be paid. Virtually every claim that I put in is rejected.
