I'm a Sizwe medical aid member, with the broker 3 sixty health, that always declines or reject my applications or Hospital admissions. 1. My Sizwe Gold Edo plan is not working for my child, due to the fact, they dont pay the full amount at my child doctor's appointments, there are always a 50 % co- payment? 2. No private ambulance payment, I have to pay the ambulance out of my pocket? 3. My child last Hospital payments come out of my pocket again? 4. I done a applications for a upgrade on my medical aid, from Sizwe Gold Edo to PLATINUM ENHANCED PLAN... the 3 sixty health come back to me and said it's declines, how in the Hell can you declines an upgrade. 5. I ask them why I can't upgrade my child to this better plan, they come back with this lame excuse, that is not even not document on my Sizwe medical aid agreement? 6. We acknowledge receipt of your Option Change Form received by us on 20/02/2020.
