Looking for UI19 form for years from Sigma Group

I used to work for the company then left 2015 , since then I've been sending emails requesting my UI19 form , they rep**** last year October 2021 requesting my ID number and I've sent it to the HR by replying immediately same day Since today i have not received my UI19 form , now tell me how someone can received his or her uif if there's company like these delaying or acting such cruelty to their ex employees where as they know it's not easy to get a job but at least one can apply for uif benefits so long at least. So disappointed to Sigma group just wondering about other people as well if they suffering to get their benefits from labour because of such negletence and cruelty from such companies.
