What is wrong with these people???

I have just found out on my credit score that I apparently owe money to Outsurance although I have been a client of theirs for almost 3 years and my monthly premium gets deducted via debit order. I urgently need a loan at my bank but do not qualify due to my "poor" credit score. I need feedback immediately!!!!
