Pension fund withdrawal/3 different Indemnity Forms!

I resigned from the company in May.submitted my documents 19 June,with an Indemnity form,then I got another Indemnity from, also completed that one...now today 15Sep I submitted an outstanding document which Alexforbes told me about, I send it through to my previous HR.Then got an email from my HR saying I owe them ANOTHER amount...which means that I must complete a thrid Indemnity Form! I wouldn't have known that I "owe" them a different amount if it wasn't for Alexforbes saying that there is an outstanding document on my behalf! mind you I am waiting for my funds and they doesn't even have the decency to inform me that I must complete another Indemnity Form! Poor Poor Poor service
