Impossible to cancel contract

I cancelled my contract in JUNE! to this day i still get debited!!!! I've asked if there's forms to complete to cancel my contract! all my emails are being ignored. it is IMPOSSIBLE to cancel this contract!!! STILL being debited for something I don't want.
