Refused to fill uif form

I was working for SGS south Africa in a coal mine and in 2019 March I was diagnosed with an occupational disease, I went to a physician specialist in July and she confirmed that I had contracted pneumoconiosis(occupational asthma) at work. I was immediately stopped to work by the company in July saying that I'm unfit to work. Then in September 2019 the company stopped my salary until in 18 December 2019 when they incapacitated me due to ill health. In February I requested my UI19 which they gave me, when I submitted to labour center I was given a UI2.7 form for illness benefits so that the company may fill showing which months the company stopped paying. When I gave them the form to fill they refused saying that I'm not an employee so they won't fill the form. So even after they fired me because I got asthma in their environment, they still refuse for me to get my uif benefits. This is exploitation of my rights. Please help Stressed and unhappy
