Urgent review requested: Pension Benefit discrepancy

Subject: Formal Complaint and Request for Urgent Investigation – Pension Benefit Reduction

Dear Pension Fund Office],

I am writing to formally lodge a complaint and request an urgent investigation into the recent communication I received regarding my late husband’s pension.

A week before my husband passed away on 28 May 2025, he specifically instructed me to contact the pension office to confirm that, upon his passing, I would continue to receive the full pension amount he was receiving. I followed his instruction and contacted your office, where I was assured that all our paperwork was in order and that I would continue receiving the same monthly amount.

However, during a call today, I was informed by a representative that only 50% of the pension amount will be paid into my account from tomorrow onward. This directly contradicts the information previously provided and what my husband and I were told and agreed upon.

I hereby request the following:

That the recorded phone calls I had with your office before and after my husband’s passing be reviewed to verify the information I was given. That our original documentation be retrieved and reviewed, specifically the section where my husband selected the option for the full pension amount to continue to me upon his death. That this matter be urgently investigated and corrected, as this discrepancy has caused unnecessary distress during an already difficult time. I trust that your office will handle this matter with the seriousness it deserves and that it will be resolved promptly.
