2 Tax numbers

It has been two times already within a space of 5 years that I have been assessed with two tax numbers. This was the case this year again and obviously my assessment is not true reflections of the truth. Tax number that they say is wrong is *** (Ref ***). They penalized me with R750 for not submitting. The right tax number which is *** (Ref ***) is the one I submitted. I asked the guy who was serving me why do I always have to have short falls rather than getting something like other people. He said is because I don't have medical aid . That's **** becausu I have medical aid for the past 23 years I have been working. Obviously they got wrong information about some of my details. I want them to re-assess all my submissions for the past 7 years as they obviously have been doing wrong assessment. The other thing is that every year I change my current address but every time RP5 comes with my previous address I resided in for the past 19 years. That's very ridiculous. I want fair and straightforward and re- assessment for the past 7 years and I want to lodge a dispute with them, that I submitted all my documents is them that check me with wrong tax numbe
