Unauthorized debit and not reversible from bank

I was debited by Sanlam Life an amount of R249.99. My bank informed me the debit order mandate was failed on the system yet the debit went through. I did not authorize this amount. The description on debit is MDFORONLON204458 and bank confirmed its Sanlam Life. I cannot get a hold of someone to help me get this reversed
