Termination of policy

I hav a 10 year investment policy with Sanlam, have been pating for 5 years now and i realised that my investment is bnot growing at all. i phoned the call center asked them to termonate it and give my money as i'ts not showing any growth at all. they promised to send me the cancellation Form and i'm still waiting. i called several times and they're telling same story but i still did not get the forms. If someone can attend to this matter urgently.
