********** policy opened in my name

There has been a policy opened under my name which I did not authorise. This is the second time this policy has been opened without my consent or authorization. It is deducting from my persal, this is now the second deduction. I have reported this but STILL it has not been stopped, it continues to deduct. This is *****!! Please cancel this policy and refund my money, otherwise I will have no choice but to cancel all my policies with you and go elsewhere, this us unacceptable.
