********** deductions on my account

There is a policy that is deducting from my salary since Oct 2017.All the details in it are not mine even the beneficiary. I want my +R200 premiums back.I have never spoken to any of their consultant nor taken this policy. I want my money back and Sanlam Sky must stop these ********** deductions.
