Duplication of A *****ulent Policy by A Sanlam Consultant who is Masiviwe Jola.

I am Thabisa Mnyaka residing in Mdantsane Nu 17 in East London. I am currently working at Department of Education Rubusana in Mdantsane Zone 3, for a EPWP Project.

In January this year, A Sanlam consultant his name is Masiviwe Jola who *****ulently opened a policy under my name, through persal where they deducted money directly from my payslip. I took all the necessary step to be followed for the policy to be cancelled, I even had to travel to Sanlam to their Vincent Branch, I was assisted there by A lady named Eve Mapela. They canceled the policy they refunded my money for full 2 months.

Now i have just received my payslip for June Monthend. The very same culprit who is Masiviwe Jola *****ulently opened a new policy for the second time, same amount, I am able to find out about that after I have received my payslip on the 20th of June 2024 that is last week Thursday afternoon.

What Must I do now Sanlam??? where are your records that shows that this policy was cancelled,Which system are you using that is not able to pick up a case that is closed due to *****. I will not suffer the consequences of the staff that you hire. I am laying a charge. I need the details of the culprit. I am definitely opening a ***** case towards the consultant who continuously opens *****ulent policies under my name through persal. I also want to find out from you, The name of the person who works for Department of Education, Salaries Department who provides my personal information to your consultant. Reason why I am seeking such information, it is because Salaries Department closed off this case with all my certified copies, I'd document, affidavit and all the cancelation forms.

The Question is... How possible a *****ulently policy that was closed in a ***** relation to be opened again whilst all the necessary documents were provided.. Signed and Emailed to you.

Sanlam I urgently need answers from You.

Regards Thabisa Abongile Mnyaka
