Before rating we would like to ask for assistance

Before rating we would like to ask for assistance . On the 1st of March I handed my resignation letter at Lenmed Wilmed Park Private Hospital. They work with sanlam for pension funds. I filled in all the forms and waited for my response, money .But when I was supposed to receive it I was told I didn't submit some of them which are called section 4 &5 .I and my HR resent them . 2 week later again I was told to hand in the same forms again I showed them proof of sending but they is still no move .I don't know what to do I really need need your help.
