BAD adviser

i had been contacted by what i belive a financial adviser is ( riaan roodman) out of the blue, aftwr the passing of my father in regards to a death benefit... The email was plain and simple, a form to be filled in the pages i need to fill in ( page 4) and i should give it to my stepmother at the funeral. I did and when i contacted sanlam ( the lady Heraldine i believe was so kind and friendly) informed me my information was not right and thati had completed the wrong form. Which i did not understand and she also saif not all the information is there... She said she would foward me the correct forms. aunfortunately my email had trouble and i believe it did not get back to me. After several attempts emailing riaan ( remember he made contact in this regard) saying what was said and asking the correct forms, it was to no avail and i am still without yhe correct i formation. Please help me as the lines ro an agent was also muddled on friday. i really dont know why there is financial advisers at sanlma not helping you and just ignoring your emails, please bear in mind they are service providers and not doing us a favour. im a beneficiary on the annuity
