Assupol Life open policy **********ly without my knowledge or permission

on the 15th April i realised on my payslip that money is deducted from my payslip R279 for Assupol life, i then called them to ask them why and they said there is a policy on my name,when i asked to see the contract the address was wrong, actually everything except my names, persal number and my ID numbers, the person who is a beneficiary is unknown to me.i then asked them to cancel the policy because its *****. they canceled the policy and sent me proof,and also refunded my money, but to my surprise on the 15 May money was deducted again, i sent them an email and they don't reply at all. i have proof that they said they canceled the policy.
