Medical Certificate

I would like to know and understand the meanig of the medical certificate lately. It seems like things have changed since i got it from RAF as part of the payout. Why do i have to go to the public hospital? When part of the payout was allocated to the medical bills. Since i am not working and can't afford a private doctor, i should use the public one because i was using it before the accident.
