Application form to take my car for wheel alignment

Good day, I have a scratch and dent cover with the provider. On that package I can do wheel alignment once a year. I have been requesting them to email me application form to get authorization to take my car for wheel alignment for the past three weeks and it is not happening. They promise to send within 30 minutes but i am not receiving the document. This is really making me upset because I am paying for the service.
