Cellphone insurance linked or added with my knowledge.

Very disappointed and angry with Jet and Hollard cellphone insurance, I once had a phone insurance with Jet store which is linked to Hollard and I requested to cancel policy due to their limited cellphone of choice upon claim in July. But in my surprise today I recieved email confirmation of a new policy with policy no. EDC***, of which I don't know who gave them consent to activate cellphone policy to my Jet account, very disappointed. And this this gives me concern because this means that my Jet account is not secured and protected if there can such policies linked to my account without my acknowledgement. I need Jet and Hollard investigate with this and give me answers!, As this might lead me to close Jat account thought have long relationship with them but this I can't take it!.
