Place debt review against your name without letting you know,

On the 10/05/2021 I received an email email from my employer to say they noticed am under debt review, my reply to them was, are you sure I never rep**** to any debt review, they send me a screenshot from my profile were it states I rep**** for debt review on the 20/04/2021.I was so so shocked I know I never app**** for any. I contact transunion just to double check they confirm debt review was place on my name on the 20/04/2021 my blood was cooking because I know for a fact I never app**** for any,a lady at transunion gave me the number for the NCR. I made contact with them spoke to a guy asking him if he can assist me by checking he advise yes, gave him my ID number checking advise yes I was place under debt review as confirmed date on transunion, am so angry he said the best thing he can assist me with is giving the people name who placed me under debt review, gave me the for RandwiseDMG, I made contact with them spoke to a lady advise me yes it was an error, how can you messed up someone else credit profile, and not letting them know if I didn't receive an email from my employer I would never had know that this debt review was place on my name.
