How can you lose documents via email

I submitted a 3rd claim to Quest Solutions. My vehicle was damaged by one of their clients. After I submitted the documents I got confirmation that 1 document was outstanding. I submitted the requested document. Now 2 weeks later I get an email from them informing me I never submitted the documents. All on email. How is this possible?.
