Rejected claim

To whom it may concern at PPS, Regarding claim MN ***/ SP CASE***, submitted due to time off work following a knee operation, I was informed that the claim was rejected based on a policy exclusion. My broker requested the policy document showing my signed acceptance of this exclusion two weeks ago, referencing MN ***/ SP CASE***. As this matter is time sensitive, I would greatly appreciate it if someone at PPS capable of assisting with this request could contact me directly.
