Requesting UI-19 from Power Fasion store.

Hello Peter I'm so disappointed on October I went to Power fashion to request the UI-19 form to claim UIF since well I'm no longer working there. Okay when I get there the manager was off and found the acting manager and told him what I want, He told me to write my details down because it takes 48 hours. I then took a taxi and went back home I wanted for the acting manager call till today. On the 19th of November I went back there remember I take 2 taxis to get to the store, okay I went there finally I got the manager and asked for UI-19 form she told me excuses at the end she asked for my details and I gave her and waited. After some minutes she said "you want the form now" and I answered yes she then say "no it is going to take time coz I'm waiting for the head office call now why don't you leave your email address so that when they get back to me I'll email the form to you" I said no problem coz it was getting late. Up until today I'm still waiting for the manager I then texted her to ask if they didn't get back to her and she said"no". I then called the Power fashion head office and told requested that form they asked me which Power fashion and I told them they then said I should email and explain to them I asked for that email address and they gave me the wrong address. Hope I will find help from you because I've wasted so much money and time by going to the store.
