Refund outstanding for 2 mths, no response

REFUND outstanding for 2 months, claim paid to the dr and not to us, the member. Phoned 5 times, Monica Molobela promised me twice that she will contact me on 2015-12-02. Also e-mailed her twice. No response. Please advise.
