Chronic doesn't want to authorize my DVT Medicine

I married my husband in 2001 and went onto Polmed with an existing DVT problem. I used Waffarin and after 4 waffarins was used my blood did not thin - and I was revered to a Cardiologist whom put me on Clexan 80mg one injection a day. That was approximately 2003 - I used Clexan until 2010 - when my doctor heard of a cheaper blood thinning medicine he revered me again to a Cardiologist whom prescribed Pradaxa 110mg x 2 per day. Polmed authorized it unlimited and a letter was send to me under our nr ***5 - 23 January 2015 again authorizing it unlimited. Now Polmed changed administrators and the administrators Chronic Department was phoned by my doctor - last week almost every day - every time something new. I emailed the proof of the letters to them - my previous conditions letters - all the proof that I was on Pradaxa from 2010/05/24 - Still they do not approve it - In between I am living with DVT which is a fatal decease case and they do not care with out my medicine. Again the doctor phoned them today - again they wanted proof of chemist - We supp**** all the documents - they just always comes back with an excuse - Polmed your administrators are bad.
