Lack of customer service

My boyfriend needs to urgently apply for his UIF benefit and requested Planet Fitness to give him the UI19 form, since yesterday. He's been calling and calling, and when he does get through to Phillip, he's promised to be given feedback. I mean how hard can it be to get the form to him??? Really! If they had bothered updating the UIF system when he left, we wouldn't be in this situation.
