Cancelled policy ITC reflects wrong

I have cancelled my policy a long time ago, by accident checking my ITC record it displays that I still pay a monthly installment of R3048.00 and this is my current balance, can my this be resolved ASAP Account Number OT ********** 8, I want this removed from my ITC record it's been more than a year now
