Placed under debt review

I have been placed under debt review in 2015 and i was not aware of it. I only found out in 2019 about this. I currently have no oustanding accounts and still debt review is under my name. As far as i know i did not sign any documentation what so ever to be placed under debt review. I need to have this debt review removed from my name as this has been causing endless problems for me and i cannot afford to have a company do this for me as i am unemployed and dont have the funds to do so. What can i do.
