Wrong credit enquiry

On the 8th of Feb 2021, a credit enquiry was made on my name by Old Mutual Finance. Upon me calling to ask who authorised the enquiry (as I suspected *****), I was told a staff member made the enquiry from old files and that the said staff member would be disciplined accordingly. I require written communication to this effect for my records.

I had then requested them to remove the enquiry from the bureaus as I am trying to repair my credit standing. Today that enquiry is still on my name with no further communication from Old Mutual Finance. I need that enquiry removed from all affected credit bureaus immediately or else I will report this irregularity to the Ombudsman.
