WHY WAS MY CLAIM REJECTED

Hello Peter. Last year around March my insurance with old mutual lasted and I was given a chance to reinstate it which I did, unfortunately on the 19 of March 2024 I lost my dad and I sent a claim to old mutual and it was declined saying that that policy had lapsed ok fine I let it go, but to my surprise every month from last year end of march to December 2024 they have been detecting money from my account for my late dad and after 10 days they send it back.so my question is if indeed it was not reinstate why are they debating money for my late dad.why did they decline my claim .Old Mutual I want answer so I have been to 3 different branches hoping I will get answer Buy no till today no one knows why my claim was rejected only for old mutual to be collecting money for my dad.
