URGENT - Unauthorized policy and deductions

Good day,

My pay-slips show that Old Mutual has been deducting money from my account for moths now for a policy I have NEVER signed for. I did not authorize any policy and was completely unaware of these deduction. Before taking further action, I want to be advised on this matter ASAP.
