Unknown Individuals added to my policy

For a few month a debit order of R303 was done on my account from Old Mutual, upon querying I found it was funeral cover for additional family members added including "my mother" now my mother passed away in 1999 already, upon doing my own investigation found that these members were strangers that were added to my account, I asked Old Mutual to investigate and correct, I reversed and stopped the debit order on my banking app, Old Mutual has not helped me only asking me to complete forms and take it in physically (in 2024) to an old Mutual office - I am not prepared to do this. this is my concern: 1 ***** committed on my account with OM, POPIA contravention because it was accessed and actioned without my knowledge and permission, 3. the lack of concern from OM, is this not a privacy bridge? has this been reported to the authority? and why is OM not assisting or taking this seriously, have the individuals been removed from my policy? has it been investigated. I have existing policies with them and will cancel because the debit has once again been actioned this month.
