Unknowingly Placed under debt review

Good day. I am not sure if this is the right platform but we have been put under debt review while asking about loan consolidation where Old Mutual Financing will pay off all your debts in one go and you pay Old Mutual monthly installments. Nothing about debt review was discussed. I can't recall the lady's name that helped us but she just continue and agree in a hurry and said it's not debt review. When we received the documents via email we saw that it was debt review through a company called Oxygen affiliated with Old Mutual. I immediately called their office in Kuilsriver because I wanted to detest it and they said the lady is not in she is the only one working with that. I then emailed a letter to them where I cancelled it in 2019. I still have the typed letter but not sure from which is shop or email I sent it. When I went back after a week to see where this lady is they told me she resigned. So I thought Old Mutual/ Oxygen received the letter. I never intended to go under debt review as I paid my debts myself but when I wanted to apply for a loan and inquiry regarding getting my own home the company's informed me that there is a debt review flag on my name which I did Not know and I contacted Oxygen which they told me I must get an attorney to get that flag off my name and I got statements of amounts I owed them ,I never made use of them as it was Nog my intention to be under debt review in the first place,we wanted to apply for s loan to pay off our debts and that is what appeared on my name and I have spend thousands on seeking help but still it remains on my name. I am not happy with this and want my name to be cleared from debt review
