Unauthorized Policy in my name / Falsified docs

Almost a year now since I discovered that there is a funeral cover being deducted straight from my salary. I've inquired at Parow Branch (Old Mutual) where i was assisted by Clint Japhets. He explained to me that I need to complete a ********** claims affidavit as the details that was submitted under my name was ****. Its now almost a year later and still nothing has been resolved. The amount deducted from my Salary is now more than R3000. Today was my 4th visit to the branch and for the 4th time i had to fill in all the paperwork. This is tiring and extremely stressfull. Ive almost given up.
