Unauthorised debit when request was confirmed as actioned for no further debits

Went to OMSA 10 July 2024 to stop a savings product to be closed as I found out during tax season 2023/2024 it was not the retirement annuity I wanted. Received confirmation via sms it was stopped, and no debits would go off. Took the actual retirement annuity I initially wanted in 2022 with another institution and come 20 July 2024 not only did the RA debit go off, but OMSA still debited???
