Unauthorised debit orders
Old Mutual team hasn’t got back to me regarding unauthorised debit orders that have been going on my account and I have a reference number

I am writing to formally lodge a serious complaint regarding the conduct and handling of my policies by Mr. Herbat Mokate.
In January 2026, Mr. Herbat Mokate assisted me in opening a savings plan and retirement plan with Old Mutual. However, I later lost my job and visited the Old Mutual branch for assistance and clarity regarding these policies.
To my shock, I was informed at the branch that the policies had allegedly been cancelled in January 2026. Despite this, debit orders continued to go off my account from March 2026.
This is completely unacceptable and requires urgent explanation and accountability.
If the policies were indeed cancelled in January 2026:
1. Why were debit orders still processed against my account?
2. Why was I never notified in writing about the cancellation of the policies?
3. Why were the debit orders not immediately stopped?
4. Why was money taken from my account for policies that supposedly did not exist anymore?
5. Why has Mr. Herbat Mokate continuously delayed resolving this matter and provided excuses instead of clear answers?
I have attached my bank statement as proof of the unauthorized deductions. Specifically, on page 3 of my bank statement dated 09 March 2026, there are two debit orders of R1000 each that were deducted from my account by Old Mutual.
As I am currently unemployed, these deductions have caused me severe financial hardship. I therefore formally demand a full refund of all monies debited from my account relating to these policies.
Please treat this matter with the urgency and seriousness it deserves. I expect a full written explanation and resolution within 7 working days from receipt of this letter.
Should this matter not be resolved satisfactorily, I will escalate the complaint to the Ombudsman for Financial Services Providers and any other relevant regulatory authorities without further notice.
I trust Old Mutual will handle this matter professionally and ensure accountability.
