Tax plan not cancelled

good day<br> <br> i had a Tax Plan with Old Mutual last year June and i cancelled the policy on the 18/11/2015 and they gave me a reference number which is CE*** and to my suprise i called the old mutual customer service to follow up on december as i was told that it only take 15 days for the policy to be cancelled but the lady i spoke to said that the form was npt filled in correctly so i needed to go to the branch to fill the form any branch so i decided to go to the Balfour park mall branch and the lady that assisted me called the head office and requested the form so i could fill the missing information which i did which was submited on the 7/12/2015 and on the 26/12/2015 i went to the branch to follow up cause when i call the call center they told me they couldnt pick up anything but the lady from Balfour called and was told that everything was submitted to the Head Office and they will pay me my money back which is 1541.90 and till now they havent paid me my money back as the turn around time was 15 working days but is now a month and no one knows what they are doing,such bad service and on top of it they still deducted the money for december.please please assist as im tired.
