surrender of retirement annuity

Hello Peter, i sent a complaint to you on 5th May 2015,regarding the retirement annuity. After consulting with you i received a call from old mutual that i will be getting my money before friday 8 May 2015. I waited and nothing happened. Then today tuesday 12 May 2015 i got a call that i must come and refill the surrender forms as they cant find them. They saying i never sign them of which i did. My problem now is im away from the branch im in Swaziland and really need the money. And all this time have been doing follow ups and they have been telling me tht i will get the money but all of a sudden there are no forms. This is really a problem for me. Kindly help please
