Risk contract cancelled with no warning

I have a client with a contract that has been active for years. Her email address was known to Old Mutual for years. Lat year the contract fell behind and she was never informed of the arrears nor was I and hence the contract lapsed. The client is not very happy and the excuses and feedback we get from consultant and call centers do not make sense and is not consistent. This is scary and shocking to say the least.
