Reinstating of benefits: Policy No. ***9

<p>My policy was recently Reinstated after it was wrongfully cancelled and I requested cancellation of certain benefits i.e. </p> <p>1. LSADDB/1</p> <p>2. PREMIUM PROTECTION (DEATH) LSD/4</p> <p>3. PREMIUM PROTECTION (DEATH) LSFE/1</p> <p>The reference for the request is CCC ********** 9.</p> <p> </p> <p>When I logged on to my Portfolio today I discovered that LSD/4 & LSFE/1 were completely removed whereas I only requested the removal of the premium protection only.</p> <p> </p> <p>I request for Immediate reinstating of these benefefits and only the premium protection that must not be reinstated.</p> <p> </p> <p>Regards,</p> <p> </p> <p>VM Mayekiso </p> <p> </p>
