Refusal To Issue Shares

After they listed, in 1998, Old Mutual sent me a form to claim my shares, to which I was entitled.
I sent this in, twice (once hand-delivering it); and both times thay 'lost' the documents.
I complained here some years ago; and got an immediate, 'helpful' response, with an instruction to call Mr X, who would sort the issue out. He was unable to help; and passed me on to an invisible, arrogant 'consultant' who proceeded to tell me where to go... I would not be getting my shares - I'd left it too late.He didn't believe that I had submitted the forms, nor was he interested in their 'promise' never to sell the share without my permission. I was easily traceable, and had never been warned that the shares could be sold.
Subsequent to this, I became very ill; and could not do anything about the matter, but I am not willing to be fobbed off by a huge institution who could lose documents twice; and not be bothered to address the problem in any ethical way.
I will not let this slide. I don't know whether I can mention names, but if it's OK to do so, I'll provide the name of the contact with whom I first dealt.
