RA policy withdrawal NO payout

I contacted Old Mutual on the 24/07/15 requesting forms to surrender my RA plan & surrender forms were sent to me the sameday (04:24 PM) from GSEnquiries. I sent the completed forms and also attached the required (ID/Bank Statements/Proof of Address) docs to GSEnquiries on 16/09/15 at 7:39 PM ***. I have had numerous calls and follow-up emails asking about the status of this request
