Poor Service Life cover confirmed in place was never done

I requested a update on my life and disability cover, received written confirmation the agent Byron Hite that cover is in place and debit order will run on 1 Oct. When following up on the updated policy documents old Mutual said my cover isn't in place yet after he confirmed it was this is unacceptable
