Policy sent to wrong people

I have a policy with old mutual so are my parents. Last year I had an issue whereby my father's confidential policies were just exposed to me, because they sent his personal staff to my postal address,sometimes I sms my fear is if I am getting their staff then clearly it could be that someone is also exposed to our staff,I addressed the issue via email and I got a confirmation that it was sorted. I even asked my dad to go to his nearest old mutual to check if my details could be linked to his profile and they said its not. Suddenly this morning I received my mother's confidential policy agreement again from Old mutual via email, is this not suppose to be confidential? Why is that our agreements are just exposed like that to everyone
