Policy opened without my consent.

Too whom it May Concern,

I am writing to bring to your attention a concerning matter, when I when through my payslip I discovered that a policy had been opened in my name without my knowledge or consent. A deduction of R530 has been made from my salary starting from April 2025. I attempemted to contact Old mutual on July 16, 2025 but unfortunately, I was not able to get the assistance need. The first agent transferred me to the group schemes department, and the second agent disconnected my call. I am requesting that the deducted amount be refunded to me and the policy cancelled with immediate effect.
